I am making this statement in relation to an incident I attended on **(DATE)** whereby at **(TIME)** I arrested a male/female whom I now know to be **(OFFENDER)** of **(ADDRESS)** on suspicion of **(OFFENCE)** following a report to police of **(DESCRIPTION OF OFFENCE)**. I was booked on as callsign **(CALLSIGN)**, the incident number was **(INCIDENT REF)** and my attendance times are recorded on the log. At the time of the arrest I was single crewed/with my colleague **(OFFICER)**.

The location of the incident was **(LOCATION)** and whilst on scene, information came to light that caused me to suspect **(OFFENDER)** was guilty of/about to commit **(OFFENCE)**. As a result at **(TIME)** I arrested **(OFFENDER)** on suspicion of **(OFFENCE)**. I then stated the caution "YOU DO NOT HAVE TO SAY ANYTHING BUT IT MAY HARM YOUR DEFENCE IF YOU DO NOT MENTION WHEN QUESTIONED SOMETHING WHICH YOU LATER RELY ON IN COURT, ANYTHING YOU DO SAY MAY BE GIVEN IN EVIDENCE." I then explained that the reason their arrest was necessary was (CODE G).

During the incident I was wearing bodycam number **(BWV REF)** and exhibit the footage as **(EXHIBT REF)**.
They were then transported to **(CUSTODY)** Police Station to be booked in to custody.
