About **(TIME)** hours on **(DAY)**, **(DATE)** I returned to my property to find that my house had been entered without my permission. I can confirm that entry to my premises was made by **(ENTRY_POINT)**, **(IMPLEMENT)**, **(EXIT_POINT)**. A **(SEARCH_TYPE)** of **(ROOMS)**. I found that the person(s) had **(SECURITY_MEASURES_TAKEN)** and had **(DAMAGE_ENTRY)**.
As a result of this I have checked my house and can confirm that the following property has been stolen and/or damaged:
