Recognition of Life Extinct (ROLE)
I am (name/rank) XXXX based at XXXX Police Station. I have completed training allowing me to recognise death in certain circumstances.
At (time/date) XXXX I attended (address/location) XXXX where I saw the lifeless body of (name or description if not known) XXXX The body was identified to me by (name and address) XXXX who stated that they were the XXXX (relationship) of the deceased and that they had known them for XXXX years.
Also at the scene were (Officers names and collar numbers) XXXX
I made the following observations of the body and scene (position of the body, injuries, signs of death, any indication of alcohol or drug abuse etc.) XXXX. I was informed that (give a brief history and circumstances leading to death) XXXX

- Decapitation
- Dismemberment to the point where life is clearly extinct
- Decomposition to the point where life is clearly extinct
- Incineration to the point where life is clearly extinct
- The body is a skeleton
- Drowning where the person has been submerged for a significant period, supported by discolouration, decomposition, bloating, absence of breathing and pulse
- The body being in a state of rigor mortis, accompanied by significant discolouration (including pooling of blood by gravity), there is absence of breathing and pulse.
